Do you think you may have, or be at high risk of developing high pressure?
The Care-Connect Hypertension Screening Programme monitors those with suspected hypertension or those with known risk factors for developing hypertension. The Care-Connect team will provide you with all you need to monitor your blood pressure over two weeks, providing the clinical team with information and data to assess your blood pressure.
You, and your GP, will receive a report outlining your results. If you are suitable, you will be provided with access to the Hypertension Education Support Programme which is delivered in collaboration with Croí.
To participate in the Care-Connect Hypertension Screening Programme you must:
1. Be an Irish Life Health member
2. This programme is currently not available to VHI members
3. Have access to a smartphone or tablet
